Welcome to The Urology Clinic!

Please review and complete the paperwork. If you have any questions about the paperwork we will gladly assist you at the time of your appointment. PLEASE DO NOT MAIL.

We ask that you bring all of the following to your appointment:

• Specialist Co Payment
• Completed Paperwork (download copies from the list on this page)
• State Issued ID
• Current Insurance Cards
• Current Medications
• Current Allergy List
• CT or other radiologic images- These can be obtained from the facility where you had the study performed and will generally be put onto a CD at your request.

We would also like to make you aware that it is quite likely that you will need to provide a urine specimen at your appointment.
